Boarding Documents

All Boarders/Patrons: These documents are to be retained and referred to by all boarders and patrons of Oak Park Equestrian Center. Please read them over periodically not only as a refresher, but also because they are subject to change as conditions do:

For Oak Park's Records

New Boarders/Patrons: New boarders, please print out the following five documents (plus the sixth if applicable). Patrons not boarding need only fill out the Release & Waiver of Liability form prior to handling any horse. 
Click on the title highlighted in blue to bring them up. Read and sign them, then return to Oak Park before or at the time your horse arrives. Horses will not be allowed on the property without these documents. You may either put them in the mailbox up at the Main Barn or scan and email them back to Contact@oakparkec.com

  1. Release & Waiver of Liability Form
    Any person who rides or handles horses anywhere on Oak Park Ranch, MUST SIGN THIS DOCUMENT. This includes friends/visitors of the Owner/Boarders and Clients of OPEC and AG Equestrian. 
    Owner/Boarders: If you plan to have someone ride or handle your horse, please print this out for them. Have them sign and return this form PRIOR to engaging in any equestrian activity. Initial each page.

  2. Boarder/Horse Information Form 
    Please fill out a SEPARATE form for each horse boarding at OPEC.
    On page 2, provide us with your desired Boarding and Feeding Program for your horse. If your horse is currently being leased, please indicate the monthly amount to be billed to the Lessee and deducted from your bill.
    The information on this form will be directly transferred onto the Stall Sheets and into Quickbooks for billing purposes, so it is important that this information be accurate.  PLEASE NOTE: If you can not drop off your forms ahead of time, please fill out and submit this Program Change Form ahead of time, so I can create a stall chart for your horse(s) prior to their arrival. Include your Phone Number and Veterinarian Information in the Comments section. This online form however, does not replace the hard copy. You will still need to fill out the Border/Horse Information Form and return it with the other documents.

  3. Board Agreement (Ranch Copy)
    Please read this document carefully and completely. Your signature certifies that you have read, understood, and will abide by the terms of this agreement. Please sign return.

  4. Ranch Rules (Ranch Copy)
    Please read these rules carefully and completely. These rules will be enforced. Your signature indicates you have read, understood, and will abide by them. Sign and return.

  5. Vaccination Policy
    This is a mandatory document for boarding at OPEC. Sign and return.

  6. Medical Treatment Authorization & Consent Form (For Minors Only) 
    This form only needs to be filled out for anyone younger than 18 years of age, who may be riding without a parent present.
